Unlike traditional dental insurance, our Dental Health Plan offers straightforward savings on the treatments you need to maintain healthy oral-systemic conditions. With no coverage limitations, no annual caps and no surprises, you can enjoy clear, predictable benefits.
Individual Non-Periodontal Plan
Includes:
- 2 exams per year
- 2 cleanings per year
- 4 bitewing X-rays per year
- 3 periapical X-rays per year
Individual Periodontal Plan
Includes:
- 2 periodontal exams per year
- 4 periodontal cleanings per year
- 4 bitewing X-rays per year
- 3 periapical X-rays per year
Individual Child Plan
For patients under age 14
Includes:
- 2 exams per year
- 2 cleanings per year
- 4 bitewing X-rays per year
- 12% discount on all other procedures
Payment Policy
Your annual membership fee is payable in full upon joining and is non-refundable once any discounted treatment or membership service has been provided.
Membership services and discounts are valid for one year from your date of enrollment.
It is essential that you keep scheduled appointments, especially cleanings. A missed appointment without a 48-hour notice will count as one of your covered cleanings.
All fees must be paid at the time of services. Any service not paid at the time it is received will be billed to you at the standard rate.
Please Note: The Dental Health Plan is not an insurance plan. It is a dental discount plan, and cannot be combined with insurance benefits or other discounts.
